Performance Detail

Priority 1: Protect and promote the health of Alaskans.
    Target #1: Number of Alaskan adults who enroll in tobacco Quitline services

Methodology: Methodology: The department has seen a national decline in quit line enrollments during the COVID-19 pandemic.
In 2021, the North American Quitline Consortium published a report demonstrating a significant decline (27%) in call volume to State quit lines during 2020 which mirrored the timeline of the pandemic. The report identified stress, anxiety, and increased cigarette sales as possible factors that could have contributed to the decrease in calls to State quit lines during the pandemic. Similarly, there was a decline in enrollments in Alaska`s Tobacco Quit Line during the pandemic.

The Tobacco Prevention and Control Program is currently evaluating Quit Line services through several methods to improve services and increase enrollment.

In FY22, the TPC program began a 3-year project to complete satisfaction surveys with 1,000 Alaskans who enrolled in the Quit Line. The results will be used to improve Quit Line services and accessibility. (Findings will be available in Fall 2024).

Statewide Survey of 2,400 Alaskans to assess system and individual barriers to calling the Quit Line Key finding - A majority of surveyed tobacco users saw a health care provider in the past year, but fewer than half were advised to quit. 68% of tobacco users who were advised to quit by a healthcare professional felt that their advice was effective.
This emphasizes the importance of ongoing work by community grantees and contractors to support health systems changes, such as universal tobacco screening and referral to treatment. Evidence shows provider counseling on quitting tobacco is an effective tool to support people trying to quit

Analysis of results and challenges: Evaluation & Management (E&M) claims were identified using CPT codes 99201-99499 and billed by provider types 020, 021 or 034 (Physicians, Health Professional Groups or Advanced Practice Registered Nurses). FY2023 saw an increase of 3.3% from FY2022, likely due to services normalizing after the end of the public health emergency. The unique count of recipients that accesses E&M services was divided by the number of Medicaid recipients that received any paid Medicaid service in the state fiscal year.

Analysis of results and challenges: The decreases seen in FY2020 through FY2023 can be contributed to the exclusion of claims processed by the Administrative Services Organization in the utilization check. A slight uptick in the percentage of Medicaid eligible individuals who utilize Medicaid can be seen in the SFY2025 number compared to SFY2024. The division expects this percentage to continue to increase now that Division of Behavioral Health claims have returned to the MMIS. The overall approach to compiling these numbers includes taking a unique count of members with a paid Medicaid claim within the state fiscal year and dividing this by the total unique count of enrolled members in the State fiscal year.

Analysis of results and challenges: Cost drivers per recipient can fluctuate dependent on a variety of factors, such as institutional rates established by the office of rate review, encounter rates established by the federal government, procedure code rates paying in a variety of ways established by the federal government, geographic location pricing established by Senior and Disability Services, just to name a few. In FY2021, there was a significant reduction of cost per recipient attributed to the COVID-19 pandemic. FY2022 reflects an increase which could be attributed to recipients receiving treatments that were postponed earlier in the pandemic. FY2023 saw a decrease in the cost per recipient as things normalize with the pandemic ending. The division expects the cost per recipient to continue to increase now that the Division of Behavioral Health (DBH) claims have returned to the MMIS. This increase can already be seen in the data with DBH claims returning to the MMIS midway through SFY2025.

Analysis of results and challenges: The counts decreased in FY2021 through FY2024 as rendering providers on claims processed by Administrative Services Organization were not included in the count. The number of providers enrolled in Alaska Medicaid in FY2025 increased to over 30,000 providers. The unique count of providers that rendered service on a paid claim remained relatively steady, with an average of just over 15,400 providers rendering service across FY2024 and FY2025. The rate of enrollment for rendering providers is expected to increase in upcoming fiscal years as new enrollment types are added, and behavioral health claims return to the Medicaid Management Information Systems (MMIS) in CY2025.

Analysis of results and challenges: The FY2021 through FY2023 participation rates are low which is due to excluding rendering providers on claims processed by the Administrative Services Organization (ASO). From FY2022, provider enrollment has slowly risen by a few hundred providers each fiscal year, while the number of rendering providers on a paid claim has stayed rather consistent with an average of just over 15,600 providers rendering services on a paid claim in the time frame of FY2022 through FY2025. The sharp increase in FY2025 is largely due to the return of behavioral health claims to the Medicaid Management Information Systems from the ASO.

    Target #1: Percent of Alaska adults with substantiated reports of abuse or neglect

Methodology: This performance measure is calculated by taking the percentage of Alaskan adults with at least one report of harm having at least one substantiated allegation. The percentage is calculated by taking the unduplicated count of adults who had at least one report of harm filed for them with at least one allegation determined to be substantiated at the conclusion of the Adult Protective Service investigation over the estimated population of Alaska for the year being targeted. There are two important caveats to the methodology used to calculate this performance measure: first, the population estimates are taken from the U.S. Census, and it encompasses entire population of Alaska, not just adults. Secondly, the number of adults with substantiated reports of harm is subject to change at any given point in time, due to variations in the amount of time to close investigations. Both of these caveats can potentially bias the overall percentage. The division utilizes its Harmony database system to store and monitor all data pertaining to Adult Protective Services.

    Target #2: Average time to initiate an investigation

Methodology: This performance measure is calculated by averaging the number of business days it takes Adult Protective Service investigators to conduct the initial face-to-face contact with the alleged victim of abuse, neglect, or exploitation once the report of harm is received by the Division of Senior and Disabilities Services. The division strives to conduct the initial face-to-face visit within 10 business days of receiving the report of harm. The division utilizes its Harmony database system to store and monitor all data pertaining to Adult Protective Services.

Analysis of results and challenges: In FY2025, the Background Check Program received an additional four positions, increasing the personnel cost and requiring upgrades to the electrical system for the additional desks. However, FY2025 saw a 32% reduction in wait time frames for provisional clearances while seeing an 8% increase in the quantity of background check requests.
